My Chosen reflection.
I have chosen to reflect on catherisation of a female done by a charge nurse from the perspective of observing as a student nurse. I will follow the Gibbs’ Reflective (1998) Cycle which is one of the reflection model cycle in nursing care. The model has six stages that explore an experience namely, description, feelings, evaluation, analysis, conclusion and action plan.
According to National Health Service Inform (NHS, 2021), urinary catheterisation is a procedure used to drain the bladder and collect urine, through a flexible tube called a catheter. Catheters can provide an effective way of draining the bladder, for both short and long-term purposes. RCN, (2021) It involved aseptic technique of washing hands thoroughly, sterilising and disposing of all used equipment, also wiping bench with disinfectant/alcohol and the use of disposable gloves to prevent cross-contamination.
The procedure started by requesting for consent which the patient said yes and the removal of the gown. While Wilhite, (2010) explains that informed consent is a process where clients are informed of the risk, benefits and alternative treatment empowering client to play a part to make informed and autonomous decisions proceed for the treatment. It is critical to explain the procedure and ensure the patient is comfortable by asking them how they feel after procedure has been done (Taleschian-Tabrizi, et al., 2015).
There were some ethical issues that needed to be considered during the procedure. Some of these issues included: sensitivity of the latex or allergy; presence of bladder stones; blood infections; blood in the urine; urethral injury, and kidney damage. As such, I felt that aspheric techniques are important to learn in nursing before you start any procedure to stop bacteria contamination (Taleschian-Tabrizi, et al., 2015). The nurse communicated with the patient by consistently asking if there were comfortable or in any pain. The procedure was successful, and it appeared to be straight forward. The patient was able to talk while having the procedure.
Regarding the feelings, I felt that the nurse had experience in this area to carry out the procedure even though the nurse was a newly qualified on the ward. I felt that it was necessary to know about the relevant anatomy before carrying out the procedure because it helped in inserting the catheter correctly and preparation process.
My evaluation is that the nurse carried out the procedure successfully. She summarised how she was going to carry out the procedure, ensured that the patient was well positioned, she reassured the patient during the procedure. It is critical to explain the procedure and ensure the patient is comfortable by asking them how they feel after procedure has been done (Taleschian-Tabrizi, et al., 2015).
My analysis is that while it is can be a complicated procedure on other patients, it is critical for all notable elements to be included. These start from indicating complications, noting any meaningful contraindications, focusing on any possible complications and so forth.
The exposure was very helpful in cementing my knowledge regarding the procedure. Despite the fact that the patient had other medical problems, she still remained composed and appreciated the nurse’s input. I was also able to appreciate how aseptic technique practices such as hand washing, and the use of disposable gloves were in preventing cross-contamination. The opportunity also helped cement my understanding why catheterising is important on a patient.
The conclusion is that I will need to continue to engage with the nurses and practically exercise to be able to perform catheterisation. The action plan is to execute when opportunity arises so I will be able to complete this task. I will take my clinical studies seriously and continue gaining clinical skills opportunities rises.
